- completion of  military service or permanent exemption card (back and front in one file)
- a copy of service commitment letter form
- Image ID of all pages (in one file)
- National card image back and forth in a file
- Scanned  photo with dimensions of 300 by 400 pixels
- Certificate of competency / training skills
- Completing and submitting the security inquiry form
